The assessment meeting is fundamental to getting the service right to meet your current needs and then a regular updated care meeting will make sure that whatever happens with your illness we will change your service to meet your needs.
The service may include all sorts of support that include remote vital sign monitoring, access to your clinical notes from primary care to help understand your medication through to Physiotherapy and other additional services. What matter is to meet your needs for safe service support to manage your illness and enable you to live as independent a life as possible.

Our charges and pricing

Hourly charges begin at £30 per hour with shorter times available if required. Travel charges per visit is made of £2.50. Please call us at the branch to ask for Pam or Amy and we will tell you aLl about the service and cover all the terms and conditions for this service.
